Sever anemia

Hello , I’m from Romania.Our cat ,Pataki was diagnosed with immune mediated anemia. He was also diagnosed with coronavirus and he has treated with GS . He was also treated with prednisone for about 3 months but the blood analysis are still bad . They have recommended us to being treatment with leukeran, but they’re stoped the treatment with prednisone 3 weeks ago. I’m very scared i don’t know what to do .

@AndreeaO how many blood transfusions have you tried? Have you tried cyclosporine? Is there an infection too? Are you doing RBC and PVC often? Are you trying liver support drug therapy? What will they eat?

@AndreeaO Is there an infection too?
Are you doing blood work for RBC and PVC often?
Are you trying liver support drug therapy?
What will they eat?
What tests have been performed on the liver?
What are the liver values in the blood work?
Has an infection been found?
Is he on fluid therapy?

I also think he is in need of a transfusion. I think the virus triggered the immune response anemia.

The virus needs to be fixed so the body stops destroying it’s blood causing anemia.

Transfusions need the same blood type from donar cat.

The anemia won’t stop until virus is under control.

You need liver function tests.

What color is the poo?
Whar color is the urine?
Has the liver scanned.
Is the liver enlarged?
Has a fine needle biopsy been done of liver and surrounding organs for cancer?
What color are the eyeball whites?
What color is the skin and gums?

Liver function needs to be tested constantly.

PCV needs to be tested ever few days with a FBC.

The COOMBS TEST shows life threatening levels.

Is the cat in intensive care??

Is he on oxygen?

Can he walk and go to the toilet?

Is he facing a wall or corner or back of the bed?

Have X-rays of the chest and abdomen, and an abdominal ultrasound examination been done?

A urine test needs to be done. A urinalysis will provide a baseline measure of kidney function and indicate whether there is evidence of an infection in the urinary tract.

X-rays need to be performed. Chest X-rays help detect or exclude the presence of cancer in the lungs, while abdominal X-rays or ultrasound help detect or exclude the presence of cancer within the abdomen.

Is the spleen enlarged? This needs to be ultrasounded.

The spleen enlarges as it finds itself processing far more damaged red blood cells than it normally does.

Poo test needs to be done for internal bleeding.

@pussiette please help us understand, did you try Leukeran (also called Chlorambucil) with your kitty? If yes, can you say if it made any difference?

@AndreeaO - if your vet recommended Leukeran I think you have to give it a chance. However, Pataki should show improvement in a few days. Testing is the best way to know for sure if there is improvement, so testing is a good suggestion. If you don't see improvement, it's fair to ask your vet about trying other medicines.

Leukeran (also called Chlorambucil) no it is a cancer treatment.

More diagnostics need to be done to find the cause of the anemia,

Hello , îs very hard for me to understand all this medical terms.
we discovered this severe anemia in December. He receive treatment with: prednisone and erythropoietin for 3 months, and we made the blood test every 2 weeks but they don’t get better.
then we moved him in a bigger city in a animals hospital.
They made a lot of investigations and they send the samples in Germany for result.
First he revived treatment with pills GS for 7 days for coronavirus. He dont have Coronavirus anymore but the anemia is still sever . The doctor wants to begin with Leukeran or Chlorambucil and the is the red cell are highest the he will receive also blood. after all that problems he is ok . He is eating and drinking all the time .

@AndreeaO hugs n positive vibes. . I did see on 3/2/21 the glucose elevated a little out of range on the lab report 142. perhaps elevated from stress?

(on a side note did vet rule out any chance of feline diabetes? (interesting to share, my pups glucose was high and anemic , on medications and drastically improved)

Pataki's blood test definitely shows a problem. I think you are doing all the right things taking him to a big city hospital and doing lots of tests. This reminds me of several years ago when I had a cat that developed lymphoma. I'm not saying that Pataki has lymphoma....this is just an example. I used to to take my cat to see the oncologist (a specialized vet) every 3 weeks. With cancer, I was told that you have to keep changing the drugs you use because cancer cells become resistant. So, my cat went to the oncologist every 3 weeks and had her blood checked, and then the oncologist would decide what drugs to give her. I must have gone to the animal hospital 100+ times but my cat lived like this for a long time; when she died it was actually from heart disease, not cancer.

The good thing for my cat was that we found out it was cancer early on. That was determined by an internal medicine specialist (I'm not sure how many specializations there are for European vets have but here there are many). After the internal medicine vet found it was was lymphoma, she sent me to see the oncologist. So back to Pataki, I think it's important to find out what is causing the blood test to be abnormal. Of course, this is the hard part.

It is also important for your vet to give Pataki "something" to restore normal blood values. I compare this to one time I had an iron deficiency and my doctor told me to take iron, but we also had to find out what was causing my iron to be low. After my iron got back to normal, I had to see an oncologist but in my case it was not any kind of cancer. Doctors and vets are always testing and saying "OK it's not this" and they test for something else. I'm sorry I can't be of more help, but I think you're doing the right thing and all these tests will help to get to the right answer.

You must be very frustrated after all of this because you want Pataki to be well. All I can say is look how he used to have coronavirus, but now that's cured. In my opinion, not as a vet but as a regular person, it doesn't hurt to try Leukeran if this is what the vet suggested. Whenever you give a new drug, it's important to keep testing so you see if the drug is having an effect. That way you can decide if you stay on it or try something else. I hope this makes sense.
